Individual Counseling (Self-Pay)

Initial session $150

Follow-up sessions $75

Weight management package: Includes 2 weeks of menus tailored to your specific calorie needs and dietary preferences based on your nutrition goals**. Also included in this package is the initial consultation and 3 in-person follow-up sessions $350 ($395 value)

Add-On Services

Photo Food Journaling: Receive feedback on up to three meals per week. User will submit photos of meals via the phone app MealLogger. Great for those looking for extra accountability for weight loss or increasing fruit and vegetable intake. This service also includes two 15 minute phone call for discussion per month. This service is open to anyone and does not require an in-person appointment. $50/month or $135/3 months ($150 value)

Personalized Menus: Up to 2 weeks of personalized menus based on your dietary needs and preferences.** This service is open to anyone and does not require an in-person appointment. $20 via email or $25 via postal service

Weight Loss/Management Support Group: Through Facebook, you can join the weight loss support group where you will receive product suggestions, advice, motivation, and recipes. I do my best to post every other day at minimum. Great for those looking for accountability and additional resources. Free

Payment is due at time of service.

 

Insurance

I currently accept the following insurance companies for weight management and general nutrition counseling:

  • Anthem Blue Cross Blue Shield
  • American Specialty Health (ASH)
  • Medicare
  • Cigna
  • United Healthcare Group
  • Out of Network

Please check with your insurance plan prior to our session to confirm you are covered for nutrition counseling/medical nutrition therapy services and that I am in-network. When calling your insurance company, it is recommended to get a reference number in case coverage is denied.

Physician referrals are required for insurance billing (unless otherwise specified by your insurance company).

ChooseHealthy Affinity Program

This program is a benefit provided for those who may not have nutrition coverage or have expended their limit of nutrition visits. In order to receive this discounted rate, patients must have a card or certificate for this program at the time of the appointment. Payment is due at the time of service.

Initial session: $75

Follow-up session: $55

Physician referral is required at time of the appointment.

 

What should I expect on my first visit?

We will go over your completed forms and what you would like to get out of the appointment. At the end of each session, we will set SMART goals (Specific, Measurable, Attainable, Realistic, and Timely) which will help you take small steps to achieving your overall goal. A follow-up session will then be scheduled if necessary.

I often bring my dog, Athena, into the office. She is extremely friendly towards adults and children alike. If you have any allergies or fear of dogs, I am happy to leave her at home for the day provided I have 24 hour notice to do so.
