Weight Loss Packages (Self-pay only)
Weight Loss package #1: Includes 2 weeks of menus for your specific calorie needs based on your nutrition goals as well as the initial consultation and 2 in-person follow-up sessions $275 ($315 value)
Weight Loss package #2: Includes 2 weeks of menus personalized and tailored to your specific calorie needs and dietary preferences based on your nutrition goals. Also included in this package is the initial consultation and 3 in-person follow-up sessions $350 ($395 value)
Note: If using insurance for payment of initial or follow-up sessions, the 2 week meal plan will be a separate charge of $10.

Group Nutrition Education Classes
$100 per session (approximately 45-60 minutes)
Presentations are targeted towards population served. They may include hands-on activities, cooking demos (simple, non-heat sourced), and food samples or food platters as deemed appropriate based on presentation topic. Topics which I have done in the past:

- Focusing on Prebiotics and Probiotics for Your Health
- The Benefits of an Anti-Inflammatory Diet
Great for senior communities, support groups, and work lunch n’ learns.
Limit 30 people per class. Must be scheduled 3 weeks prior to presentation date.
What should I expect on my first visit?
Once your appointment is scheduled, I will send you two forms to be completed prior to your visit: nutrition assessment and diet history. I can send these forms via email or postal service. You will also receive a reminder slip with your date and time of the appointment and directions to the office. If you are paying with insurance, a physician referral is required. To obtain a referral, I can call your doctor’s office on your behalf to request a referral be faxed to me or you can call them and have them fax it to me at (844) 364-8548.
On the day of your visit, please bring the nutrition assessment and diet history forms as well as any lab results such as glucose, cholesterol, triglycerides, etc. that you want to discuss with me. Some lab results can be improved through changes in the diet. If you are diabetic and would like to be seen for a diabetic diet consult, please bring your blood sugar log and meter if you have one. Upon your initial visit, you will receive a Notice of Privacy Practices and will be required to sign consent forms before pursuing nutrition counseling.
We will then go over your nutrition assessment and diet history as needed and discuss any questions you may have. At the end of the appointment, we will set SMART goals (Specific, Measurable, Attainable, Realistic, and Timely) which will help you take small steps to achieving your overall goal. A follow-up session will then be scheduled if needed.
