Group Nutrition Education Classes
Includes nutrition presentations targeted towards population served, hands-on activities, cooking demos (simple, non-heat sourced).
This service works well for 55+ communities, support groups, lunch n’ learns.
Limit 30 people per class. Must be scheduled 3 weeks in advance.

What should I expect on my first visit?
Once your appointment is scheduled, I will send you two forms to be completed prior to your visit: nutrition assessment and diet history. I can send these forms via email or postal service. You will also receive a reminder slip with your date and time of the appointment and directions to the office. If you are paying with insurance, a physician referral is required. Upon setting up your appointment, I can call you primary care doctor to get this for you or I have a form you can give to them yourself.
On the day of your visit, please bring the nutrition assessment and diet history forms as well as any lab results such as glucose, cholesterol, triglycerides, etc. that you want to discuss with me. Some lab results can be improved through changes in the diet. If you are diabetic and would like to be seen for a diabetic diet consult, please bring your blood sugar log and meter if you have one. Upon your initial visit, you will receive a Notice of Privacy Practices and will be required to sign consent before pursuing nutrition counseling.
We will then go over your nutrition assessment and diet history as needed and discuss any questions you may have. At the end of the appointment, we will set SMART goals (Specific, Measurable, Attainable, Realistic, and Timely) which will help you take small steps to achieving your overall goal. A follow-up session will then be scheduled if needed.
